Baby Boutique Agreement

Cascade Women's Health is concerned with safety and makes every effort to ensure all items in the Baby Boutique meet safety standards, though it cannot guarantee the quality of items that are donated. I, the undersigned, will examine items for myself and refuse or return them if they are unacceptable to me. I understand that it is my responsibility to make sure the items that I use have not been subject to a safety recall. I will not hold Cascade Women's Health, its volunteers, employees, agents, successors, and/or assigns liable or responsible if any problem should arise with any item I purchase.
